Laboratory, Medical, and Precision Equipment Service Intake Checklist
Laboratory, medical, dental, veterinary, calibration, measurement, and precision equipment requires careful intake because service decisions may affect accuracy, contamination control, patient or operator safety, records, and regulatory obligations.
Identify the equipment and its use
- Manufacturer, model, serial number, asset number, firmware, software, and approximate age
- Equipment type, installed options, accessories, probes, sensors, fixtures, consumables, and connected systems
- Intended application, operating range, environment, workload, and required accuracy or performance
- Current calibration, preventive-maintenance, certification, warranty, and service-contract status
Describe the issue
- Exact alarms, error codes, failed checks, drift, repeatability problems, contamination concerns, unusual noise, leakage, or physical damage
- When the issue began, whether it is intermittent, and whether it followed relocation, cleaning, power loss, software changes, maintenance, or consumable replacement
- Reference standards, controls, samples, or verification methods used to recognize the problem
- Troubleshooting, cleaning, resets, parts, calibration attempts, or vendor support already used
Prepare records and evidence
Provide nameplate photos, display screenshots, logs, test reports, calibration records, service history, manuals, environmental readings, and clear images of the affected area. Remove patient, customer, research, account, and other protected or confidential information before sharing files.
Control safety, contamination, and authorization
Remove equipment from service when continued use could produce unsafe, invalid, or misleading results. Follow facility procedures for decontamination, biological or chemical hazards, radiation, pressure, lasers, high voltage, and controlled access. State whether the work requires an authorized service provider, traceable standards, documented calibration, certification, validation, or manufacturer approval.
Choose the service path

On-site service is often appropriate for installed systems, controlled environments, equipment requiring facility utilities, or work that must be documented at the point of use.
